To schedule an appointment, call (614) 525-3719. Required childhood immunizations are available for $15 per shot for children ages 1 month through 18 years of age. Cash or check, Medicaid, CareSource, Buckeye, Paramount, UHC Community and Molina will be accepted; however, no one will be turned away due to the inability to pay for childhood immunizations. Please be sure to bring your insurance card, photo identification and your child’s shot record with you to the clinic. A completed consent form is required if a parent or legal guardian is not present at the clinic.
